Figure 1: Clinical course of the patient. Thrombocytopenia occurred on the ninth day after initiating hemodialysis with the anticoagulation heparin, with a platelet count nadir of 42×109/L. The patient’s D-dimer increased from 1.44 ng/mL to 8.27 ng/mL, and fibrinogen dropped from 4.9 g/L to 1.6 g/L. On the ninth day, he was diagnosed as HIT, and heparin was discontinued. Argatroban was administered and monitored with the activated partial thromboplastin time (APTT) of 1.5- 3.0 times baseline value. His D-dimer and fibrinogen returned back to normal level within four days and a gradual platelet count recovery was observed.
